Weird the AC-5E that came with the N95 has 5.0V, the ACP-12E has 5.7V.
N95 can safely be charged from ACP-12E. Its even in the user manual of N-95.

The difference of voltage or more precisely the output current is for quick charging. Its not suppose to affect the phone or battery in any case.

Faulty battery?

You could try a new battery,i'd try for a new N95 first under the warranty.
