1) yes, you do need a twitter account.
2) unfortunately you always should be worried when using the net if you don't have a data plan/flat rate for your mobile. twibble downloads the tweets of your friends and direct messages as returned by the twitter api. Given that a tweet may contain up to 140 characters you may end up with some traffic. A (yahoo!) map is around 50-100k I believe. Once the tweets are downloaded twibble only get the newest from twitter (via the If-modified-since http header). And it's always a good idea to check if your network connections have been closed properly after you quit an application that accesses the net.
